Your appeal for us to share our [malfunction or accident] stories with a larger audience (“Gearing Up” by Executive Director Ed Scott, April Parachutist) not only resonates, it makes sense. To help push people to do this, how about working through the S&TA network? If someone rides a reserve, needs help walking in from the landing area or is talking loudly in the hangar after a jump because they are amped up on an extra-large dose of adrenaline, the S&TA walks over with an incident report form in hand and asks them to fill it out (and helps, if necessary). Perhaps the S&TA can also show the instructors where they keep the incident report forms and ask them to help?
